Tier-4 leading indicators are proactive indicators that identify weaknesses in safety management systems so that they can be analysed to prevent the occurren- ce of level 1 or 2 Process Safety Events (PSE).
At Cepsa, the following tier-4 indicators are among the most relevant:
· Updated unit risk assessments and implementa- tion of recommendations.
· Compliance with established plans for safety and prevention training, drills, safety walks, preventive observations and housekeeping.
· Review of process safety procedures according to defined plans.
· Audits of work permits, technical changes and deviation-free relay books.
· Safety inspection rate (number of Preventive Safe- ty Inspections (PSI) carried out against target).
Implementation rate for corrective and preventive actions carried out.
